1. Soft tissue gingival mass causing reactive expansile buccal bone growth: gingival biopsy recommended
A soft tissue mass originating at the gingival margin has extended into periodontal ligament space at the mesiobuccal aspect of tooth 21. This mass has caused reactive periosteal bone formation at the alveolar crest at the mesiobuccal aspect of tooth 21. The periosteal bone formation in this region is reactive to the soft tissue lesion rather than a sign of an intra-osseous lesion.
A biopsy of the gingival soft tissue at the mesiobuccal aspect of tooth 21 is required to determine the nature of this lesion.

These axial cross-sectional images demonstrate the normal appearance of the root of tooth 21. This tooth is not endodontically compromised. A localized widening of the periodontal ligament space at the mesiobuccal aspect of the coronal third of the root of tooth 21 is evident in these images.
Coronal views of tooth 21
These coronal cross-sectional images demonstrate the mild localized bone loss at the alveolar crest at the mesiobuccal surface of the root of tooth 21.
2. Maxillary sinus mucositis
Mucositis is present in the right maxillary sinus. This is an incidental finding with no clinical significance.
